Leading U.S. Virgin Islands Hospice, Census 57 For Sale

US Virgin Islands, US
Asking Price:
$2,250,000
Sales Revenue:
$2,414,960
Cash Flow:
$664,028

BUYER REQUIREMENTS, PLEASE READ FIRST: Hands-on hospice operating experience (or a credible plan for experienced local leadership) and proof of funds are required with every inquiry. All-cash offers receive preference.

The leading hospice in the U.S. Virgin Islands. Medicare-certified, serving St. Croix, St. Thomas and St. John, past the 36-month rule, offered as an equity sale. Asking $2,250,000 (about 3.4x SDE). Seller financing available.

The largest hospice in the territory, protected by Certificate of Need, with 27 years of reputation, a census of 57, and a growth engine that has never been switched on. The owners are ready to retire and are committed to a smooth, hands-on transition.

Why this one is different:

  • The market leader, with the gate closed behind it. The largest Medicarecertified hospice in the territory. The USVI is a Certificate of Need jurisdiction, so a new competitor would need a CON of its own, and in a market this size that approval is unlikely.
  • Census that does not quit. Census averaged 52 to 56 patients every year from 2021 through 2025 and stands at 57 today (October 2026).
  • Strong earnings at a value price. About $664,000 of seller discretionary earnings over the trailing twelve months, and about $587,000 of adjusted EBITDA even after funding a fulltime marketer and successor leadership pay.
  • Growth nobody has tried yet. Marketing spend was under $1,000 over the last twelve months. Every patient came through reputation and referrals. A dedicated outreach role, funded in the numbers below, is the obvious next step.
  • Runs day to day without the owner. Two RN directors already manage clinical operations, staffing, referrals, compliance, and prebilling review around the clock.
  • Built for a platform. About $130,000 a year of outsourced finance and HR, plus payroll processing, folds straight into a larger operator's shared services.
  • Transfers cleanly. Past the 36month rule, so the Medicare certification conveys through an equity sale with no new enrollment, unaffected by the federal enrollment moratorium.

Earnings, TTM to September 30, 2026: net patient revenue $2,414,960; reported EBITDA $337,169; add owner expenses $127,205 and owner salary $199,654; SDE $664,028; less outreach role ($65,000) and incentive pay ($12,000); adjusted EBITDA $587,028 (24.3 percent margin). Q3 2026 annualizes to about $738,000 of SDE. FY2025 reported EBITDA was $556,017 on $2,713,991 of revenue.

Deal terms:

  • Equity sale, cashfree and debtfree. Preclose Medicare cap liability remains with the seller.

Who this is for: experienced hospice operators, regional platforms, and investors with a plan for local leadership. Inquiries must include hospice operating background and proof of funds. Qualified parties authorize an NDA to receive the confidential information package. Ref #33398123083.
