Internal Medicine Practice With 653 Medicare Lives For Sale

Flint, Michigan, US
Asking Price:
$700,000 Furniture / Fixtures included
Sales Revenue:
$609,878
Cash Flow:
$231,989

653 Medicare patients. Nine in ten seen in the last 12 months. In a county where 7 in 10 Medicare beneficiaries choose Medicare Advantage.

This established adult primary care practice in suburban Genesee County, Michigan has served the same families for more than 40 years. The panel is the asset: 889 active patients, of whom 653 are Medicare – 369 Medicare Advantage members and 284 traditional Medicare beneficiaries. 91% of those Medicare lives were seen in the trailing 12 months. Many patients have been with the physician for over 20 years, and only one or two a year leave for another doctor.

WHY VALUE-BASED CARE BUYERS SHOULD LOOK - 653 Medicare lives, 73% of the active panel - 369 MA members across eight plans; BCBSM Medicare Plus Blue is the largest - Genesee County MA penetration of about 70%, well above the national average - An engaged panel that comes in, so annual wellness visits and chronic-condition documentation run through visits that already happen - Asking price equals $1,072 per Medicare life.

UNTAPPED CAPACITY The physician-owner now works about half his former schedule, has stopped taking new patients and sees existing patients half as often as he did five years ago. The office is built for two providers and runs with one. A buyer who restores the prior visit cadence, reopens the panel and adds an NP, PA or second physician starts with a full schedule and referral sources the seller says are ready to send new patients.

FINANCIALS (tax returns) - 2025 gross revenue: $609,878 - 2025 seller’s discretionary earnings: $231,989 (38% margin) - Collections reported close to 100%; fewer than one no-show a month.
OPERATIONS Two cross-trained full-time medical assistants (23 and 5 years’ tenure) expected to stay. In-house lab with automated chemistries, urinalysis, A1c and glucose testing. Telehealth in place. No hospital call; a hospitalist group manages inpatients.

FACILITY 1,892 sq ft, five exam rooms, two provider offices, in a 2006 Class B medical office building with additional space available for expansion. Leased month-to-month from an unrelated landlord; long-term lease expected to be available.

TERMS Asking $700,000: $560,000 cash at close plus a $140,000 seller note (9.5%, 10-year amortization, 36-month balloon). Accounts receivable excluded. The physician will support the transition.

Pricing reflects the value of the senior panel, not standalone earnings. Ideal for primary care and value-based care platforms, MSOs, ACOs, health systems and health plans adding lives in an MA-dominant market.

A confidential information memorandum is available after an NDA.

Business Operation

Management type:
This business is owner operated.
Expansion Potential:

Restore the former visit cadence (patients now seen half as often as five years ago). Reopen to new patients; the seller reports referrals waiting from current patients, nearby specialists and local businesses. Add an NP or PA (wound care) and a second physician; adding a female provider would allow office gynecology. Expand telehealth and FAA pilot exams. Join a physician organization for wider network access. Launch a website (the practice has none). Bring the 284 traditional Medicare lives into value-based programs.

Competition / Market:

Genesee County, MI: about 70% of Medicare beneficiaries are in Medicare Advantage (CMS, March 2024). About 54,500 residents aged 65+ live within 10 miles. Adult primary care supply is shrinking: the seller reports that three nearby internal medicine practices closed on retirement, and another solo internist is 92. Patients come from surrounding suburbs and from up to three hours away. The practice sends patients to urgent care daily for lack of open slots.

Reasons for selling:

Retirement. The physician-owner founded the practice more than 40 years ago and wants to hand his patients to a buyer who will care for them for the long term. His spouse, who works part-time in the practice, is also retiring.
